How to Report a Problem
WIC is committed to supporting you and our WIC-authorized grocery stores. Your feedback is important to us. If you experience any issues, please let us know so we can work together to address and resolve them.
WIC Program and Participant Complaints
A WIC Program complaint can be related to appointments, customer service, courtesy, fair treatment, answering telephones, clinic hours, WIC EBT card, privacy and other related WIC service processing concerns. These complaints are investigated by the Louisiana WIC State Agency.
WIC Vendor (Grocery Store) Complaint
At Louisiana WIC, we value our participant compliments and opinions. Click the button below to fill out our Vendor Shopping Experience form, where participants and staff can tell us about your shopping experience at a Louisiana WIC grocery store. Submissions can be positive or negative.
WIC Civil Rights Complaint
A WIC Civil Rights Complaint results when an individual believes and indicates they were treated differently because of their race, color, national origin, sex (including gender identity and sexual orientation), disability, age, or reprisal or retaliation for prior civil rights activity in any program or activity conducted or funded by USDA.
WIC Program Appeal
A WIC Program Appeal can be submitted by an individual who feels he/she has been unfairly denied, terminated, disqualified from the WIC Program or required to make a cash repayment for improperly issued benefits, program fraud or abuse.
WIC Participant Fraud
WIC Participant Fraud may include but is not limited to the following:
- Dual Participation which includes:
- Receiving WIC benefits from more than one site at the same time
- Receiving WIC benefits in more than one State at the same time
- Making a verbal offer of sale to another individual or allowing someone else to do so
- Posting WIC foods or benefits for sale in print (in newspapers or publications) or allowing someone else to do so
- Posting WIC foods or benefits for sale on the internet (through websites such as craigslist, Facebook, Twitter, eBay, etc.) or allowing someone else to do so
- Making false or misleading statements or intentionally misrepresenting, concealing or withholding facts to obtain WIC benefits
- Attempted or actual returning or exchanging of WIC EBT card/CVBs or supplemental foods for cash, credit, non-food items, non-WIC foods or WIC foods in amounts other than those listed on the participant’s WIC EBT card
- Redeeming WIC EBT card/CVVs that were reported stolen, burned, damaged or lost.
- Giving away WIC Food Benefits or WIC EBT card/CVVs to someone other than an enrolled participant.
- Altering WIC EBT card/CVBs in any way.
- Other actions involving WIC EBT card CVBs in violation of Louisiana WIC policy
